Improving Quality and Patient Safety

While participation in cardiac rehabilitation programs is associated with a 20-25% reduction in mortality, often patients drop out of hospital-based programs due to long travel distances and other factors.

Toronto Rehab developed a six month at-home rehabilitation program custom-designed to support people who cannot attend traditional on-site programs.  Patients receive ongoing support from a case manager via telephone, home visits, email and via internet modules and coaching. 

In addition to dramatically increasing participation levels, the in-home program increased participant’s cardiac fitness by an average 20%.  Almost 90% of enrolled patients complied with telephone consultations and 80% participated in internet coaching modules.
