Contact Information for selected Family Health Teams

Each of the groups announced on April 15, 2005, selected to proceed with Family Health Team development and implementation planning, is currently determining the services, programs, staffing and physical resources that they require to serve the needs of the patients and clients in their communities. The Ministry has received written consent from a number of Family Health Teams that have agreed to provide their contact information in the event that members of the public, other stakeholders, health care providers, etc. would like to contact a selected FHT in their community.

Please note that not all Family Health Teams have consented to release their contact information. The Ministry of Health and Long-Term Care is obligated to protect the privacy rights of all Family Health Team applicants. Therfore, only those contacts listed below are currently available to the public.
